Only set the power to fan to 12v, like all standar fan with 3-4 pin, if your psu gives you 12v you cant go over 12v...

You have (had) a PWM fan, meaning one that controls it's own speed. The green line is the RPM sensor, and you fed it straight 12v power. Thats why it's fried.
