According to their site, both the 4870x2 and GTX 280 models, operating with fans at full speed are considerably more quiet than the stock coolers at highest speeds:

I got mine today, it runs at 70% with an idle of 40c with the stock cooler being 30% and idle around 60c and I can't hear a damn thing, I hear air moving air now, not just the sounds of fans, I can also hear my HDD's.

This thing is truly amazing, well worth the money.

I will say one thing though, it messed up my cable management as I did have to remove the middle HD cage to get the sucker in there (This card is now 3 slots big, and about half an inch longer then the standard 4870X2 is), and since the cooler covers up the 6 and 8 pin power connections on the card, they give you these 180 degree power cables that just look horrid.
