Have you considered the 5750? Performance seems right around 4850 levels- but you get the DX11 and lower temps/power consumption.

Here's a little bit of info on my ventilation: My GPU idles at 50C, max 61C at full load. CPU idles at 41C max 57C at full load. Case temp 25C max. Ambient temp 21C.

Provided you case does hot sit on carpet...
You could likely add a 120mm fan to the front bottom panel (or floor of the case)to improve your air intake. That will compliment the rear 120mm fan.

Or add a side cover 120mm fan.

Yeah, I may do some cutom drilling to add a vent to the top too. When the budget allows for it I may upgrade to the mATX Centurion 5 case.
