Ok guys. Ive already figured out where the prob is. As I remember, I havent touched my GPU and did nothing of it except that I reinstall/install new drivers of it like 8.4 driver version with CCC MOM WDM and stuff. And I think its in the drivers I installed and using Driver Cleaner Pro. When I run Driver Cleaner Pro, I read the readme instructions and it sez there running cab cleaner is not needed but recommender so I exclude running cab cleaner and proceed to the steps. Right now followed the DCPro steps again and this time I include running cab cleaner and it fix it. Its not so messed up right now. Heres the screenshots.

Do you know how to fix this prob? Cause the reset graphics processor feature of Catalys CC doesnt work on this one and its really annoying this 2 huge black lines appearing on my screen. This DCPro software really f*cked up my GPU, I hate it. I should did reformat my harddisk and install new drivers instead of using this DCPro.
