Yeah, it's weird at first, but because it's basically just a mouse flipped on its side, I got used to it in about an hour.
I have no idea how it works for gaming, but I love love love it for work!

I am curious if you settled on anything?
__________________I have the same issue. My MS Explorer 4.0 is slowly dying (left click is getting unreliable) and I would love to get a new mice. Today I went to Best Buy and looked around. They had a pretty good selection of mouses, but most of them were in boxes so there was no way to try it. After visually comparing different mouses it seemed to me that Razer Mamba was the largest. I hate wireless mouses, so at first I though it was not an option, but then I saw that it also has a wired mode. So I got it. Have been using it for about 30 min and ... it's not too great. I think my cheap MS Explorer feels way better. So I probably will return it and will look for something else. I guess the next thing to try it MS SideWinder X8. I hate wireless mouses, but it seems that I pretty much have to have one of these now days to have a big mouse. Another option is one of Logitec models. G500 looked smaller then Mamba - but who knows maybe it'll fit better regardless. There was another Logitec model - non gaming and wireless - probably MX Revolution (I did not see model's name). It was not huge, but was rather comfortable.
