Four 500GB Velociraptors in RAID-0? That's sequential performance that you normally only see coming from SSD RAID.

4 command timeout - not much, but it is something. All other Error stats are at zero. Can you run an extended SMART test on that drive to see what it says? (Pass/Fail)

Did you ever solve this one? It seems like you've taken most of the logical steps. I doubt it'll help, but check for BIOS updates and stuff anyway - if it detects sporadically or not at all in the BIOS... you never know what's causing...
