You may have just hit the limit of that chip. Mine wont budge passed 4.5. I boot into Windows fine at 4.6 but I cant pass OCCT. Heck, I was able to run Intel Burn Test for a full 20 passes at 4.8 but cant get passed OCCT at anything...
On the stock heat sink, youre not going to get much higher than 4.0 if that far. Those Bulldozers put out a lot of heat and while the stock AMD heatsinks are pretty good, theyre not going to be enough to handle any serious overclocking....
Yeah a quality 650'ish power supply would be good. Corsair, Seasonic, Kingwin, Enermax and XFX are my "go to" brands.
The 8350's are hungry little beasts so youll want as beefy a motherboard as you can get. Ideally youd want one with 8+2 phase as thatll have an easier time with it. If you could scrape up some more cash, you would be much better off. ...

Im pretty sure its your RAM. The symptoms sound like RAM issues but it not being on your motherboard's list makes them even more suspect. Time for an RMA or maybe pick up a couple other sticks that are on your mobo's support list. ...
Hadnt heard of any problem with Crosshair V's having a high failure rate. Oh well, Im good with just saying the Sabertooth rules. Its always been my favorite.
