Seems similar to my issues, however a downclock worked so not really sure. I assume you tried to reinstall it?   I eventually took my cpu down to 4.15 and my gpu down as well, now never crashes.

Well I dropped my 580 down to 900/1800/2100 and played for about 30 - 40 min last night without an issue.    Seemed to level temps as well, before I was well into the 70's now im in the 60's..    My locks...
