Default Read the voltage

So is there a software that reads the GPU voltage after a hard mod or from a bios flash?

I know HWMonitor reads my voltage, but will it read the correct voltage if I do a pencil mod?

And how about Rivatuner?

How do you read it on a 4850 (vcore and vmem) with a multimeter? I only used my multimeter to pencil vmod my ga-p35-ds3l
