Use IOBit Smart Defrag, it does a thorough job and has a light footprint.

JK Defrag, apparently now MyDefrag as of latest version, is also lightweight and very thorough. http://kessels.biz/JkDefrag/

Incomplete defrags are usually due to fragmented system files and/or insufficient free space. AFAIK, none of the free defraggers can defrag all the system files. You can try the free trial version of a commercial defragger such as Diskeeper 2009, it should be able to defrag everything; the demo version works just like the full retail version for a month.

For complete defragging of EVERYTHING, you'll need either PerfectDisk, Total Defrag 2009 or Diskeeper.
