Video card, without a doubt. You might as well get an nvidia card since crossfire isn't an option.

If you are really loading 3 gigs of mods in FO3, then the stutters might a lack of RAM.
__________________On my setup, XP + FO3 at max everything + minimal mods needs a little over 2 gigs. It would stutter a bit at times until I upgraded from 2 to 4 gigs RAM. Now it's perfect when maxed. I'm using the same processor @3.0 GHz, a single 8800GTS, and a smallish 17" monitor. Assuming your GPUs are already overpowered: Get a SSD to boot from and install to and 2gb more RAM.
