Do cables need to break-in, aka, use them for a while, then speed increases?

It could make a small difference if the cable is around a source of interference...but if its STP, it won't matter.

no.. its alot more than that. they braided different and cat6 can tell where you have a bad spot in the cable

Side question. How can you tell if a cable you have is Cat5/Cat5e or Cat6 if it's not labled on the cable itself?
