I usually do the hardest on FPS and racing but other types usually one bellow the highest.

I always start at default, then after a couple of levels, i adjust to a harder/easier difficulty if possible.

Hardest, then back down if it's too much (only happened on Mile High Club in Cod4. So much trouble beating that at first on veteran!)

What I do to make my single player games more enjoyable is whore the levels to the max. Say, if I'm in a village in Crysis, I cloak, run out, kill a soldier, uncloak, hide, cloak again, go out the other way, throw a grenade, etc etc. Take my time; I might take up to half an hour just clearing out a village.
The first time I played Crysis I would just cloak and speed past the random groups of soldiers/villages and it wasn't as fun as what I'm doing now.

lol FAIL. cal has been dead for almost a year now, if you wanted to lie about what league you are in you could of just said cevo p or boe invite like everyone else does (i find it funny seeing people with no xp saying "oh yeah i was cevo-p on my old steamid")

First play through always on normal, to get a feel for the game and of course to pay attention to the story! (I play action/RPGs alot)
Notch up the difficulty for a second playthrough, try a different style, etc

I usually play on medium-hard, I like the challenge but I hate felling frustrated when I'm supposed to have fun with a game lol.

It depends on the game, and if it is designed to be played through more than once (or if it offers an incentive for more than one play through).
If I've heard it's a pretty linear game with no real incentive to play it more than once, then I'll play it on the hardest difficulty to make it last longer. Games with one ending, no point/skill/weapon upgrade systems etc would fall into this category. Games that revolve around character development that utilise mutually exclusive upgrade paths or have multiple endings, such as fallout 3, I'll usually play on the second hardest difficulty setting first. Simply because doing that allows me to play through it the second time on the hardest difficulty - playing through the same game a second or subsequent time is always easier, and at least with the difficulty raised a notch it may still be a challenge. I just love a challenge The harder a game is... the more I enjoy it (99% of the time). I found TWEWY still a bit too easy even on ultimate, chaining the maximum amount of battles/combos at once while controlling two characters.
