I don't like having things running in the background, and if I want to play a different song, skip a track e.t.c. I have to alt+tab between windows which is slow and annoying

yep. standalone player ftw. I use winamp and the keyboard shortcuts.

ctl+alt+pg up for previous song
ctl+alt+pg dwn for next song
ctl+alt+home for play/pause

ya be sure to enable global hotkeys. this doesn't work in all games

Most keyboards tend to have programmable keys which can be used to change song without interfering with the game.

Most Logitech boards have this capability. As for FLAC in GTA4, i'm afraid it's a no go
