Your CPU is definitely bottlenecking. GTA4 like high clocks, specifically on the first core or thread. Even with my 2500K at 4.5 the game still maxes out the first core on my CPU while the other 3 are at about 50% usage, so this is why some people might think its not that CPU intensive, because if you look at a pure CPU urage percentage it will not be that close to 100% but you first core will be.

When I went from a Q6600 @ 3.6 to the CPU I have now it made a big difference but the game is still not lag free.

IF you are not having any other issues with other games, this case is solved.

That resolution could be too low...

On a second thoughts... not too low to give you 20 to 30% usage, and without a 100% CPU.

Well... now go for the basics... update to latest drivers... latest game version... update with windows update to everything's latest version... and test again

Now this post makes some sense - if this first thread thing is real. (I didn't know about this first core taste of this game)
So.. GTA 4 actually benefits from faster CPUs THAN more cores?

If this is the case, you could use windows' task manager to change the CPU priority and affinity of GTA 4... try it, OP...

I got the same CPU usage and the game felt about the same at 3.2 as it did at 4.0.

Frame rates are better but still dips down to mid to low 30s at some points GPU usage is higher but not higher than 50ish.

you need to crank up your OC there, if you find the default settings not to your liking, bear in mind the game doesn't run too well though, so
GTA 4 runs well now after patch 1.0.7. I get 65+ FPS modded with AA and texture packs at 1920x1200.
