A reset in the bootloader should get rid of any unlock codes... maybe android was designed to keep lock codes after a wipe? Nonetheless, if its phone related Mr.Pie's solution should fix it, if not it could be your sim card/phone combination thats causing trouble.

Damnit, I only have mini-usb cables lying around, no micro-usb ones. Guess I'll have to pick one up tomorrow and try it then.
