Overclocking i5-2500K - Do I have a dud or am I doing it wrong?

Thread Starter 
I've decided to jump back into OCing after switching motherboards. I can hit 4Ghz fine, but 4.5 its impossible. I tried to 1.40v and it freezes up within seconds in Prime95.

The only thing I change is the voltage and multiple, maybe I need to change something else?

Try increasing the PLL voltage to around 1.89

Several things you can try:

Raise PLL voltage as said above,
Enable Internal PLL Overvoltage (ignore the wording: This has NOTHING at all to do with the PLL voltage and doesn't raise it one bit (you can verify that with a DMM), but it's needed for a low mhz wall, where vcore no longer helps; usually allows about 500 more mhz until you hit a brick wall).
Enable loadline calibration (if you didn't enable this, that 1.40v could be dropping down to 1.30v)
