How to Contact the CPU Properly

I know that CPUs come with a metal plate over the top,

so what I want to know is, should you remove the metal cover on the CPU and contact your cooler directly with the chip, or not?

Never. The metal plate thing is a heatspreader, it's there for a reason. Removing that will limit the amount of heat transfer to your heatsink/waterblock/whatever.

A lot of CPUs have their IHS soldered onto the die now, instead of using thermal paste.

In the past, some CPU IHSes were easy to remove because they used thermal paste. It's possible to remove a soldered IHS, but it's a lot riskier.

Please beware that without the IHS, the cpu will not be able to withstand much pressure. That means the core can crack then you mount the heatsink...

The reason I ask this is, your GPU is in direct contact with the heat block over it, so why shouldn't the CPU as well? Or should there be a cover on the GPU as well and I'm just missing it?

It works when there's not much pressure and limited space.
Laptop cpus don't have covers
ATI GPUs don't have covers
nVidia GPUs have covers

You're perfectly fine.
Some overclockers go further than lapping the IHS and just remove it completely.
It's extremely risky and not recommended though.

How about for liquid nitrogen cooling and extreme sports like that? Would it be more common in those locations?

But when you can upgrade the default heat spreader to somthing better, for instance copper!, it would be more efficient to dissipate the heat. I don't see a problem with this. However, like others have said, it's probably more vulnerable to fractures.
