116 is generic for gpu, could be anything gpu related.

How are your cards connected, v1 v2...?

0x116 = Low IOH (NB) voltage, GPU issue (most common when running multi-GPU/overclocking GPU)

Are your gpu's on separate rails?

CPU/GPU #1 on one, GPU #2 and everything else on the other

bumping up the IOH at all results in an instant crash

I've gotten a 116 when my vrms overheated on my 460s. I put a fan on them and they went away.

when i run them both by themselves, there is no crash... so don't think that is the issue

When you instal new drivers do you use driver sweeper?

I ran 2 highly OC'd 480s, a 9800 GT for PhysX (also OC'd), i7 930 @ 4.4 Ghz, 6 HDs, 2 optical drives, 8 case fans, fan controller, H50 with 2 more fans, sound card and a RAM cooling fan on my HX1000w for about a year and had no issues whatsoever, even during stress testing of my cards' OCs.

It's possible it could be the PSU, but I have my doubts.

Hx1000 should have zero problems running your setup and some.
