I have it, and it works great. IRRC I play GTA IV maxed out, except for view distance and detail distance I think. I'm on the newest patch, I would play more but I want to find a good car pack. Check out this site, it's a featured mod:

I don't think it impacts performance much, but it conflicts with Afterburner so Afterburner must be closed out while playing with the mod. I would check FPS but I don't know how.

If anyone can recommend a good real car pack, I would greatly appreciate it. I will also test FPS if someone tells me how.

EDIT: Getting FRAPS to go test FPS now. Will report back.

I don't know if there's an in-game command to check FPS in GTA4, because I don't have it. However, you can download the trial of Fraps and it'll tell you the FPS counter. It won't matter that it's a trial because you won't be recording.
