Freezing/Black screen w/ xFx HD5970 2gig Black Edition

System specs:
5970 XFX 2-gig Black Edition
AMD Phenom X6 1090T Processor 3.21 GHZ
ASUS Formula IV
8 Gigs of G-skill Flare memory
COOL MASTER Silent Pro Gold 1200 watt

Ever since I bought the 5970 I've been plagued with random freezing while in games after anywhere from 5 minutes to an hour of game play. (games effected - SC2, Bloodlines, BFBC2, BF3 alpha )

After it freezes it sits there for about 10 seconds and then the screen goes black and my monitor says no input detected. The sound of the game normally cuts after the freeze but if I'm in vent most of the time I can keep talking for about 5 minutes before it totally locks up and I'm forced to hit the reset button.

Also, sometimes during the freeze if I mash control-alt-delete / alt-f4 / alt-esc / alt tab I can get to task manager and when I get back to desktop it says my video card driver recovered from an error. But then sometimes it crashes again.

Things I've tried:

Uninstalling video card driver, then using driver sweeper in safe mode and then reinstalling driver (v11.7)

Turning down graphics in game (turning down the shadows to low fixes the freezing in some games but not all. I don't really want to do that though since this card is a beast.)

I messed around a little bit with voltages of my ram but I saw no change.

Total reformat of computer (I did this about 3 months ago didn't help at all)

Any help would be greatly appreciated, I am at a loss for what to do next.

Sounds like it could be overheat condition in the GPU or VRMs if its not driver related. Have you turned the fan up to full speed to see if it still does it?

I haven't tried turning up the fan but I have run gpu-z and other heat monitoring programs and they all say the video card isn't hot when it crashes.

As for drivers, I use ATIMAN uninstaller to remove drivers with an admin account. Then use CCleaner to remove registry entries and delete AMD & ATI folders from the HDD (I only do this with Intel CPUs though). Also make sure Afterburner is not running. It causes issues too, so some uninstall it.
