shadows.

in that order, but if you are multiplaying you want shadows as it helps to see if someone is crawling or crouching and comming around the corner and you see then from the shadow

It depends somewhat on the game ... in some games certain effects are crushing on FPS (like ADoF in Metro) whereas the same effect in a different game (bokeh in JC2 is the same basic thing as ADoF in Metro, with WAY less perf hit) may not hurt you much at all. But in a very general sense, these are 'heaviest' effects:

1) Higher levels of AA ... dropping AA to 2x is probably the first thing I'd try, as 2xAA still works very nicely on most games.
2) Ambient Occlusion, aka HBAO/HDAO/SSAO. Turning down the quality of those, if that's available, or Off if not, usually helps quite a bit w/FPS.
3) Draw/View Distance. This varies a lot depending on game, but in some games having a high view distance (ArmA2 for sure is one) can be crushing on FPS.
4) Shadow Quality. Again it depends a lot on the game, but in some game maxed out shadows can be very demanding. Typically the visual difference between Medium and Maxed isn't *that* noticeable unless you really look, and it can be a significant perf boost to lower them to Medium. Other games, shadows just aren't that tough even maxed out.
5) Post-Processing Effects. This 'catch-all' category consists of a number of different fancy effects (one of which is Depth of Field, which is often it's own setting, as is Motion Blur) ... which specific effects they are talking about depends on the game engine. Having PPE maxed out can, in some games, be pretty devastating to FPS, other games, not so much. Typically these effects only occur when certain things are going on (explosions, for instance) so if the problem is just occasional lag when a lot is going on in the scene, this is a good one to look at dropping ... if the option is there.
6) Texture Quality/Size/Resolution. This one is really only important if you're exceeding your vram (not going to happen on your 6950 2gb) capacity ... aside from dropping res/AA levels, its about the only other way to drop your vram usage to a manageable level. Otherwise if vram isn't a problem, maxing out textures doesn't really hurt your FPS much.

And of course, there's also resolution. Personally I'll never play on anything but Native, so it's not an option in my book ... but it's way up there on the 'list of things you could lower to get better FPS'.

Anyways, those are the biggies, most of the other options besides these you'll see in various games probably won't make a big diff in perf, unless it's a Post Processing effect like DoF.

Lastly, there's PhysX on the games that support it. This can be a big perf killer in some games, for sure.

Hey if you keep the fan at 40% no wonder it's hitting 91C... You should get it to increase with temps... here's mine for example:

< 40C : 40%
40-50 : 55%
50-80 : 70%
80-90 : 85%
90-up : 100%

Card has never got hotter than 72C

How about instead of turning down graphics options, you actually fix your graphics card.

Is it a reference or non reference? If it's a non reference design, which design is it specifically? Changing the thermal paste is going to result in a nice drop in temperatures. Worst case scenario I would buy another GPU cooler. Also what is your ambient and case airflow like?

It is helpful because high temps are symptomatic of a ventilation problem. Why degrade visual quality and not attack high temp issues at the source-that is poor airflow?

Ambient Occ. then shadows, then water effects, then the other effects. It seems to be running oddly hot, try leaving the side panel of your case off and tipping it horizontally, This might allow more open air to it, and if you have bad airflow, then it would possibly fix it, you could then move your fans if that was a problem.
