From the situation he's describing it's a CPU bottleneck. The mere fact you claim a 3 ghz Phenom II wouldn't bottleneck a HD 6850 without even mentioning the resolution suggests you are one of those people who don't really understand what a bottleneck is.

Well, the bottleneck really depends on what resolution he's playing on, and what settings he's using. If he's on low settings with a 1440x900 res (Just using as an example and guess as most 19 inch monitors are 1440x900) then it's very likely that he has a CPU bottleneck.

If he was on high settings with a 1920x1080 resolution, then it would easily be a GPU bottleneck instead.

Not really a bottleneck, but close... it appears your CPU clock is randomly dropping mid-game causing random sudden FPS drops. A bottleneck is more of a constant thing, not a random occurence. This may be the CPU overheating or the VRMs overheating.

It is also highly possible that the RAM usage is maxing out (2GB on Vista...... yeah)

2GIG memory on Vista and that 8800?

Dude you need to upgrade. Even jumping to Windows7 would help tbh.
