When does heat affect performance?

My GTS 250 hits around 75C on stock clocks when playing BC2 and GTA IV. This is probably because there is only a 1 inch gap between the top of the card where the fan is and the bottom of my case. I also can never tell if the decrease in frames per second is caused by temperature or increased activity in the game.

Increased activity. If it was heat, then the card would drop clocks and you would see a big drop in fps or stuttering.

It won't decrease it to anything noticeable until 100+ probably. I try to keep mine below 90 for obvious reasons, but it shouldn't affect it enough that you could tell until it starts to throttle, which usually isn't until 100+. Now life span, that is a different story. The lower the better of course.
