HD4870. Games crash and things go to hell!

Hello! So... I'm using an ASUS ATI HD4870 1gb 256bit video card. I'm having an issue where sometimes my games will crash and then everything goes to hell. Let's say I have Guild Wars and Final Fantasy 11 open, Guild Wars will error and close, and FFXI will stay open but it's box will start flickering and run slow. On top of that, Chrome ends up getting an error saying something is wrong and a lot of times when this happens Windows Explorer locks up and has to be closed. Sometimes this happens within a few minutes of having a game open and sometimes I can play games for hours and it never happens.

Soooo... any ideas what's up?

CPU: Phenom II X4 955 3.2ghz Black Edition (underclocked to 2.8 due to motherboard limitation)
Motherboard: Biostar A780L
RAM: DDR2 2gb and DDR2 1gb
PSU: 500w
Video Card: ASUS ATI HD4870 1gb 256bit

Have you run any stress testing on your GPU to make sure its stable? I've never played Guild Wars but I do know it doesn't take much to max out FFXI. One thing I would look in to though is your RAM. I'm not 100% but I think you want matching sticks.
