People with keyboards don't bind every key on the board to an ability, they use easy to reach keys and modifiers. At least I do.
I've wondered this too...back in my Ultima Online days I actually wore the "Alt" off of my Alt keys on a couple key boards. I didn't think that was possible, but I suppose I was pressing it and then spining my thumb on it near contantly.

I've wondered if console controllers can only send a signal from one button at a time. This sounds absolutely insane, but it would explain it. I know my Playstion 1 did not have this trouble as games like Resident Evil would have you hold the finger button to draw your weapon.

Maybe they can't "combine" buttons? Like R1 and "Circle" can be pressed together, but can't be viewed as R1+Circle? Though again...I can remember Playstion 1 games that saw this as a different button. However, I've been playing a lot of Playstation 2 games on an emulator recently and I don't think I've came across a game yet were I press two buttons together to perform a different function. I've played FF10 and 12, Front Mission 4, Wild Arms 3, Grandia 3, A couple different Xenosagas, and Star Ocean 3. Not once can I remember holding a button down to make another button do something different; though I wasn't looking for it, but if it happened it was just a once in a "bluemoon" thing.

Any one care to comment if a PS3 or Xbox 360 can combine buttons?
