It is connected with a hdmi dvi cable straight from the videocard into the back of the TV.

If this is your card as posted by Windfire, it doesn't have HDMI output. Or you are using DVI-HDMI Active Adapter?

This is what the ends of the cable used look like.

Also I know the cable works, because a screen is being displayed on the TV when both the Monitor and TV are attached to the videocard at the same time.

I see that you are using a DVI-to-HDMI cable as circled by red in the diagram below. This diagram is page 16 of the User Manual of your HDTV.

Do you also use the stereo mini plug conversion cable for audio as well?

Check this one thing:- Get into Nvidia Control Panel and choose the resolution to be 1360x768 @ 60 Hz (orange oval). Also, if possible, try 59Hz as well.

Looking at the card outputs in picture, both DVI-I are Digital and Analog capable. And you are using DVI Digital passive adapter to HDMI which is capable of 1920x1200.

Other than setting in NV CP as suggested by Windfire, try to swap them around if it will change anything.

I already set the TV to those settings. Also the only refresh rate option is 60Hz, there are no other choices.

I had read through the user manual and followed the directions it gave to set everything up. I have gone through every resolution shown in the manual one by one. (Setting it while both TV and monitor are attached, and then trying just the TV after shutting down and restarting the computer.)

Like I said in previous post, it could be the way how the card distributed the signal internally thru two DVI ports when 2 devices are connected, but I couldn't find any reference for your specific card. Windfire will come out with something.

Edit:

It's like you need a dummy plug for the DVI-HDMI to work correctly by itself.

You can try using the other monitor cable but without the monitor attached to the cable. See if the DVI port only need a certain pin connected to route the signal to DVI-HDMI.

See if the cable will do the trick.
