Found this on official forum:
I had the exact same issue, turns out the nvidia driver had a a Rage profile with Vsync set to "Force off" by default. You may not see the profile until you uncheck "Show only programs found on this computer", then you can just set it to use the global setting or whatever you desire.

So the above worked for me but I had to set to 4x anti-aliasing, turn v-sync on then swap back to 2x for v-sync to stay on.

Use Nvidia Inspector/CP and make sure "Force Vsync" is DISABLED. ID TECH has always supported vsync at the command line: start Rage with the following parameter "+set r_swapinternal 1". This should work with any ID TECH game. And that's the reason you can't find many options in the Video pane of the game: you can set everything in the game with CL parameters or in-game with console commands! To use the console, you have to "+set com_allowconsole 1" and make sure you don't have the to date newest patch, were console support is removed. CL parameters still works though.
