Card problem or driver problem? or other?(Gtx580)

So recently Ive been having some problem with my card and/or the drivers, I have an msi gtx 580 lightning edition and 90% of the time when i exit a game i will get a weird crash and a black screen (no bsod or error code) and the sound will skip and freeze
(if it makes any difference when i get back to windows the more info at the unexpected shut down will tell me the bsod code as 116)

I'm wondering if this is a card failure or driver issue(280.26)... or something else

p.s its not overclocking Ive tested it in heaven (2.5) for 12 hours with a max temp of 56c (So its not a heat issue)

did you ever ran that card with a previous driver that worked fine? if so i would recommend re downloading that driver and continue to use it. sometimes new drivers come with bugs only to specific card. if it is the same driver you been using for a while you could find the newest driver and download that. hope it helps! good luck
