Alright, this is really annoying... another BSOD after 1hr prime 95 with +0.06 offset and previous settings... why does this keep happening? Is it possible that my chip just sucks?

Not every chip is the same. Saying that your chip sucks is way to early in this game. You are trying to get 4.5 @ ~1.3v. There are obvious enough people who are running 4.5 @ 1.35 or sometimes higher. Some people can't even boot into windows beyond a multiplier of 42 (those chips suck).

What I try to say here is, don't quit to early or be negative. OCing is a time-consuming process if you want a proper stable OC.

How long did you run prime95 with your manual vcore? Did you ran any other stress applications?

Thanks, that was actually encouraging enough to make me do another test, which I think works now.

Multiplier: 45x
LLC: High
VRM: 350
Phase Control: Extreme
Offset: -0.05v
C1E, C3, C6: Auto

Stable 3.5 hours now, and my voltage hovers between 1.312 and 1.320 when under Prime95 load & 69c max temperature!

I initially set the LLC to Ultra High with +0.05 offset, but for some reason that made my Core Voltage spike to 1.36v-1.37v... so I set it to high and now it works perfectly.

Ok this sounds great mate! The voltage you have now: 1.312 - 1.32 is not bad. Most of the time people need between 1.3 and 1.35. Keep on stability testing and watch the behavior of the vcore and temps as much as you can.

Just crashed again after ~6.5 hours of prime95 on +0.05, so I made the offset +0.10... should I just leave it at this or test it some more?

Also, could C3 and C6 stepping being enabled be the cause of my crashes? And should I change Duty Control from "T. Probe" to "Extreme"?

Use this as a template and increase the voltage to help with stability. You can move onto offset voltage once you have determined the voltage required for your desired overclock. As you increase the multiplier, test stability, you may have to enable PLL overvoltage if a mulitplier doesn't boot into windows. Work your way up from here.

I've already determind that 1.32-1.33 is sufficient, it's just the offset that is tricky. For example, Ultra High LLC causes my voltage to spike to 1.36v without a negative offset. And for some reason if I use by per core the voltage also spikes as opposed to setting each individual core.

I think the settings I have now are going to be perfect, it's just that now it hits a max of 74c when under Prime95 and I don't want to get it that hot for a full 12 hour test. Since it lasted ~7 hours before a 0.05v increase, could I just leave it as it is, or what is the worst that could happen?

Some of the settings which munaim1 posted can help getting more stability but it's different in many situations. For example i'm running 4.5 at the moment perfectly stable and I only changed the LLC, voltage and multiplier. LLC ultra high.

You can try the settings or you can try to lower your negative offset a little so for example from -0.050 to -0.040
