Artifacts with new 6570

My friend bought a HIS 6570 with the passive heatsink last night. It running on a phenom ii x2 555 on asrock 780l board with 4gb ddr2 and wd blue 500gb hd, antec 550w neopower psu.

He had been using nvidia 8500gt and unistalled the nvidia drivers in safe mode then installed the 6570 and installed 11.9 ccc from the amd website. When he games or in steam window she is seeing diagonal lines streaking across the screen. He is using the vga port which comes from a ribbon on the card.

I had three thoughts, not sure:
1) Interference due to the unshielded vga cable. I told him to try it out of the dvi port or hdmi port but he needs to get a cable.
2) Bad driver install.
3) Bad card artifacting.

Any ideas / suggestions are appreciated.

Here is the card on newegg.

Sounds like incorrect settings. Try remove driver > shutdown > move the card to different PCIe slots > install driver > check refresh rate (Hz). Or try DVI-VGA Adapter.

Gave him one earlier but it has the 4 analog pins whereas the dvi output on the card is only dvi digital so the adapter doesnt fit. Seeing if we can find another adapter or cable.

Tell him to go into CCC > VGA Display > check EDID if CCC set the correct resolution and refresh rate. If not, manually set the correct value.
