Speakers / Headset , Push to talk / voice activity?

This was for WoW, but I guess any online game would do. I'm trying to figure out how other people talk and play well....
I use a desktop microphone and I push to talk when I raid or pvp, but the only time I can really do that is if I'm not moving around so much or if I have time to auto attack : /

For those who raid and/or pvp, I have a few questions for you:

1) Do you use a standalone desktop microphone and speakers to play or a gaming headset with a built in mic attached? (if headset, USB or jacks?)

2) What voice chat program do you use the most?

3) Do you use push to talk or voice activity? (if push to talk, which button is assigned?)

My goal is to be able to play freely while communicating with my teammates How do you guys do it?

I have a really cheap Logitech headset with a built-on mic, it has 2x 3.5mm jacks, the clarity is absolutely fine.

I've got it so the game audio comes out of my speakers, and ONLY the voices of my friends come through the headset. Makes it easier to hear people as the game and voices aren't coming from the same place.

I also use push to talk, and use one of the side buttons on my roccat kone+. I despise people who don't use PTT.

I've used Roger Wilco, Teamspeak and Ventrilo and have to say that I actually enjoy using skype the most. It's clearer (for some reason) and it seems easier to use than the others, despite the others having more options for fine-tuning.
