Could you show which temperature is reaching 100°C? It should throttle when reaching that mark.

Clean up the stock paste and apply with new thermal paste such as MX-3/MX-4. Then try fan speed @100% and watch the RPM is actually at full rev.

I remember reading about the nVidia Sensor Reset Tool which deleted some registry entries or some such which could mess with accurate temp readings.

These two, in the ControlSet:

RmThermalProviderNum
RmThermalProviderInfo

I've no idea if this would work with AMD cards as well, and if it'll even fix your problem seeing as this 'solution' assumes the sensor itself isn't malfunctioning. Figured I'd at least let you know about this. Try at your own risk.
