Latencies in games have been horrible, around 1000ms or so. Webpages take a few minutes to load. Youtube videos take 15-20 minutes to buffer a 1-2 minute video.

Hmm, couple questions. How long have you had your modem/router? And which isp are you on?

The 'Request Time-Out' doesn't mean there is an issue there. It simple means that gateway, router or server doesn't respond to pings.

Those are likely still within your ISP.
The link to your first hop seems to be fine, likely congestion within the provider.

That latency is horrendous.
