Screen goes black

Hey guys I keep having weird issues,

Sometimes when I watch tvcatchup on my pc I get weird issue, where I lose the video and both screen go black as if the graphics card would be turned off(blinking light on monitor). I can hear sound but there is no video output sometimes the sound gets stuck in a loop. And now playing Darkness 2 demo the screen went black as if there no input to it and the sound was stuck looping.

Anyone has this issue before?

Yes my 570 Was overclocked for a period of 2 months now it's running on stock, my I5 2500K is currently overclocked at 4.0Ghz

I'm continually experiencing this issue on my 4870x2 (stock clocks). It usually happens in 3d mode. Basically, the more graphically intensive the task, the more often it crashes. On certain occasions it can run without crashing for quite lengthy periods of time, though.
After I reset my PC, I get the notification that my system had the BSOD 116. Dumps usually indicate atikmpag.sys as the culprit. I have tried flashing the bios in order to disable 2d downclocking, tried increasing the voltage a notch, tested a bunch of different drivers, inspected the card for physical damage... Nothing helps, sadly.
In your case, however, all might not be lost. I have read a lot of input on this problem and here are some points to consider:

1) Whether there are any conflicting drivers. Have you used any AMD card on your system previously? If so, have you COMPLETELY removed any and all traces of the drivers? Have you tried different nVidia drivers?

2) Whether the cooling is sufficient. Could your card be overheating, by any chance?

3) Is your card overclocked? You said that is no longer the case. If so, what were you running at previously? Was the voltage increased? Have you tried downclocking yet?

4) Does your system create crash dumps? If so, have you tried analyzing them? If you haven't, could you post them here for us to have a look?

5) Is usage of that "tvcatchup", or whatever, thingy the only occasion your screen goes black? if not, what are the circumstances of the other crashes?

6) Have you tried stress testing your card after the problem started? E.g. some good ol' furmark burn-in?

1- Nope got a brand new fresh install since my old SSD died on my so i got the new crucial m4 (i had the black screen issue with my old SSD too)
2 - Cooling seems good never goes above 68C
3 - With my old overclock everything was increased, both Vram, core and shader, but that was only for a week then i did an overclock but without changing touching vram or the code voltages.
4 - Nope no crash dumps because there is no crash, I just loose the video output from the card because I can still get perfect sound, but sometimes it does have the lopping sound meaning it did crash. All I do is just hold the power button to turn the pc off.
5 - "tvcatchup" is a website to watch live TV, it does crash in firefox but not in chrome, i guess it's because fifefox has hardware acceleration. It began doing it in games too and when I did the set up to enable nvidia 3D.
6 - nope haven't done any stress testing.

My computer> Properties> Advanced system settings>Advanced>Startup and recovery>Write debugging information>Small memory dump

This makes the system create minidumps once a crash occurs.
When I crash, on certain occasions the sounds keeps playing as well... For about 5-10 seconds. Then it either starts looping or stops completely. Since your problem seems to be escalating, I get the feeling it might be a hardware fault. Not to get you down, but in the 4870x2 threads, people often ended up RMAing their cards once such stuff started happening.

Still, let's not give up without exhausting all available options first.
Can you remember when the issue started? Perhaps you had recently updated the drivers back then? Try reverting to older drivers, maybe even some ancient ones. Driver issues were some of the most widely discussed in the 4870x2 threads, and on the occasions when the troubleshooting succeeded, it was the drivers that had usually been at fault. I suggest assuming faulty drivers for the time being, and see where that leads you.

EDIT: by the way, here's some background info on the issue (I'm writing from memory, so if I make a factual mistake, please correct me):
When this sort of problem occurs, the cause is the graphics processor becoming out of sync with the instructions provided by the driver (usually, the GPU lagging behind/hanging). When the discrepancy becomes too great, the driver attempts to reset the graphics processor and get it to a proper working condition. However, if it fails to do so in a set amount of time, it, sort of, "gives up", which then results in a loss of display output, hence the black screen. If it succeeds, we get the "blah blah blah driver has recovered blah blah" error.

Yeah on that part you are right, windows will try to reset it and recover it, I had the message before on my old pc but not now. It first happened a month or so ago while watching tv on firefox, i could hear the sound but the screen was just black.

Is there any software to test the gpu for artifacts or errors?

Furmark is a great tool for stability testing. Be sure to crank up the fan speed, though - the load it puts on the graphics card is tremendous, which translates to a massive temperature increase.

Is it really in the 16x slot? Hmmm... One of the GPUs on my card runs at 4x when the drivers are not installed... As said previously, try experimenting with different driver versions for now.
