Just got mine. This is all it came with and it looks brand spanking new.

Mine looked brand new too, plagued by the Ventrilo robot voice now though(nothing against the sound card, it can happen to anyone). I'm just using on board for the mic instead. Sound quality sounds awesome for music, haven't watched any movies yet.

Using Audio Creation mode with bit-matched playback enabled and master sampling rate at 44.1 kHz

It has pins on the card to connect to the cases mic and headphone jack. It doesn't come with the creative front panel attachment.

Got mine today, also. Got ddl going (after figuring out I needed to set output to "speakers" and not the digital connection) and it appears to support dts connect in vista/7 according to the drivers.
