Overclocking Help- New and Having Issues

I just recently built a rig for my girlfriend so that we could play some games together again after her old prebuilt system blew up. The specs for her new system are:

i5-2500k
asrock extreme 3 gen3 z68
8gb 1600 corsair ram
corsair 850 txm
h60 (going to be push pull, need to get the screws to attach second fan, unless im retarded and missing how to hook it up)
msi 560ti twin frozr ii (not yet in the system, arriving 2moro)

So we just bought the h60's from work, we work at best buy and I was excited to put em in and start trying some overclocking so that I could learn and then work on mine. I am no OC professional, the extent of my knowledge is how to push the oc genie button on my msi board. I have done my research though and looked at serveral guides. I believe I disabled all that needs dissabling, and dropped my PLL close to 1.7. I put my volt to 1.25 and raised my multiplier up to 45. From everything I read, that should have been an easy jump, however, it didn't work. I have moved it up to 1.285 and still it turns on, but instantly has a core fail in prime95. My temps are no where close to high, and I'm unsure what could be wrong. The multiplier is messed up sometimes when it shows in cpuz, not sure if thats an issue. Any help would be greatly appreciated. I was hoping this would be a system that would last, moving to potentially an 560ti sli whenever I upgrade my video card.

more voltage! The volts you are talking about is for the processor right? Only 1.285 volts for a 4.5Ghz OC is wayy too low. In the 1.4s is what is normally needed for that kind of clock.
