Random blue screens/shutdowns/graphics drivers failing

The following problems in the title have been happening a lot more recently, ever since my SSD broke down. I had to reinstall Windows on my normal HDD, and things have been kinda glitchy since.
Although when simply browsing the net these things can happen, they normally happen when playing League of Legends and WoW. The following happens:
a) Blue screen of death with the BAD_POOL_HEADER error b) The screen randomly turns black for a few seconds saying "DVI no signal" on my monitor (it usually says this when my computer starts up) then goes back to normal c) My computer shuts down without a blue screen after a brief freeze
Any help?
EDIT: This morning I got another blue screen, this time with the error MEMORY_MANAGEMENT

Assuming that you haven't OCed any of your hardware, then this issue sounds like a bad HDD. Run checkdisk on your hard drive and see if it finds any problems. Since this wasn't a problem on your SSD, then I'd bet money that the problem is software related. Worst case, reinstall Windows and see what happens.

*Though one thing you can check hardware-wise is that the HDD is properly attached. Disconnect and reconnect the SATA cable at both ends as well as the power connector.
