IIRC in ME2 they said the reapers built the relays.. that makes little sense. Even if they did, who built the first reaper(s)? Who actually built the Citadel? Why on Earth were the Normandy crew in the middle of a MR jump? Why were they even leaving the solar system?!
The only one that's a plot hole is the Normandy and as stated before, the current idea is that shepard was indoctrinated.
Obviously someone built the reapers and they were the first AIs torebel and kill their creators.
Soverign stated in ME1 that 'they' created the mass relays and the tech everyone uses.
The rest of what you describe isn't a plot hole so much as its simply not explained yet. They could have made the catalyst first and he could have made the reapers. They could have made a basic AI like the geth who then built a more advanced catalyst or reapers. Who knows.
