What fan are you using? my gt ap-15 is 1800 RPM but doesn't push as much air as fans usually do at that speed. I can not remember my distance, I think it might be 1.4-2 cm but you can judged from my new picture I have put up. I did not double check my result as I had seen one on the net some years earlier with 2 fans, were getting 12 degrees difference. Also my room temps were very low 17.5 degrees  and I have good air flow in my computer.

Edit: it seems less mayby 1 to 1.6 cm?

Event log is clear, checked that last week when I first installed the card and had issues. Seeing how it only happens when vsync is enabled I feel like it must be a driver issue.
Results (in progress)
Dirt 3 @1440p, max settings, max AA
no vsync: 98-99% Usage, 67 min fps, 80+ avg, full boost to max overclock, no stuttering just screen tearing.
vsync: 70-80% GPU usage, boosts to full overclock all the time, stutters down to 56.
Sorry to ask a bunch of questions, but you have me curious about something now.
I am not sure if you're running afterburner, but if you are, does the stuttering by chance look similar to this on your hardware monitor? If so, I think I may have been wrong and have experienced similar stutter with high AA. I almost forgot about this.

My card will not boost past its set clock shown in GPU-z unless I OC.
I would like to check the BIOS version beings I may have found something that may explain why my card will not boost anywhere past set boost clocks like everyone elses card. My BIOS on my card is 80.04.09.00.C0. Some other screenshots I have seen of the Twin Frozr GPU-z from a few reviews that I could find have a BIOS version of 80.04.09.00.8A. So far just about all GPU-z screenshots in this thread have a different BIOS version than mine. That is why I am thinking it may be BIOS related why my card acts differently. And the reviews I have seen for the Twin Frozr all said their card would boost past what was set also.
Anyone know how they do naming for BIOS versions? Like would the .C0 that I have technically be newer than the .8A that review cards had?

I maxed at 1137.1

Sorry, I didn't have time to do any more tests, but it does look similar to that.

I don't run afterburner, but the dips are similar as they are only for a split second, but noticeable when you are dipping more than a couple of frames.

It happens at almost any setting though, not just high AA.

Even games like Fifa 12 which are simple to run stutter like crazy, but some are better than others. D3 beta was decent for example, while nba2k12 is unplayable and sometimes lags to the 30s for no reason.

This same setup was playing everything perfectly with sli 570s a couple weeks ago, so it's not something crazy, it's just the card or the drivers. I'm hoping it's the drivers since I just installed a waterblock on my 680 yesterday assuming it was the drivers lol.

Heres a link to a thread on the nvidia boards about the problem:
