I'd say just in the Nvidia Control Panel and off in the in-game settings, since it overrides the game's settings, and it's the GPU that will be implementing this Adaptive V-Sync, not the game. Enabling V-Sync in-game has caused some problems for me in a few games (Dead Space 2 anyone?), so off in-game and on in NVCP seems like a good way to go.

Seems to work great in BF3 for me.
I don't know were to find Vsync in the V control panel though.

Adaptive Vsync is BAWS for 120hz screens. Disabled Vsync and AA ingame for most of my games and enabled FXAA and Adaptive Vsync globally. Every game runs so much better. FXAA does look blurry though in some games and makes text look a little bit weird, especially in League of Legends but there was no way to enable AA in that game before anyway. The Half refresh rate vsync option works wonders in Skyrim as well. Adaptive Vsync doesn't seem to work in BF3 and Guitar Hero though which is a pain.
