Ahh. Yeah, the buzzing is because the drivers locked up the entire computer, and its sending one little clip of the audio it was just playing over and over. Honestly, the simplest way to find out if its a driver issue is to replace it with an identical card, same drivers, see if the issue repeats.
Drivers have to be such low level programs able to directly and efficiently access the hardware that its very hard to tell for certain whether the problem is the drivers or the hardware, because they're so closely linked. A hardware malfunction can easily confuse and crash a driver if it doesn't get what its expecting, or it could be a bug in the software itself. There are signs you can look for though that would point toward one or the other.
Points toward hardware malfunction:
-Artifacts on the screen. Most often caused by an overheating or otherwise malfunctioning GPU. Can also be caused by a loose DVI cable, so check your connections as well.
-Any sort of smell (obviously)
-Black marks/warping of PCB
-Inconsistent video performance. Usually if a problem is caused by software, it will be working one moment and completely dead the next...there is no in between.
Points toward software malfunction:
-Sluggish or buggy graphics control panel.
-Settings don't save properly, or don't work at all
-A hard lock of the OS (this is what your video shows)
-Repeated display driver crashes (with no hard lock)
It's also important to note that you can have hardware failures that look like software failures. For instance, if you have bad sectors on a hard drive or bad memory then those will all look like software problems. But they will also usually come with their own set of symptoms.
