The modem is the device that will "convert" the fiber (or COAX) connection to a standard Ethernet one, just like your ADSL modem "converts" the phone line to an Ethernet one. In most cases, it will be a device similar to what I assume your current router/modem looks like.

Long story short, since you will be switching from ADSL to Fiber, they'll need to supply or offer a new device anyway. This device should support the correct speeds of the service natively, and you may even have the option of getting a unit with wireless and gigabit capabilities built-in.

No, you can't use your upstream link to receive data, although that would be a sweet hybrid implementation to have one dedicated download stream and have another half duplex link to accommodate both up and down.

A modem simply facilitates layer 1/2 connectivity to your ISP. IP assignments are a result of their DHCP server.

You would be fine on the 100 mbps package with a router that has 10/100 ports as long as there is a fast enough CPU to route 100 mbit worth of packets.
Something like the RT-N16 is rated around ~140 mbps WAN to LAN, if you cheap out on a crappy device you likely will not achieve the full throughput.

If you can afford 150mbit net, you can afford a gigabit router and/or switches. 100mb/s will bottleneck that connection hard.

just get a gigabit router and ensure you use CAT5E. we had the same issue at work with our 240mbps of BW. we had all the speed but it wasnt running right, turns out the firewall maxed out at around 60mbps

Either that or build a firewall/router box which should have enough CPU to deal with your throughput needs. How many people are behind your ISP connection? Something like untangle may be a good solution that will handle the total throughput.
