$1 transaction fee plus 15% whenever you cash out into Paypal. No 15% fee to keep your funds within Blizzard but you can only keep a max of $250 and only buy Blizzard products or DIII items with that money.
So in theory you can sell a ton of items and only have the $1 transaction fee until you hit that $250 cap and at that point take a 15% fee to transfer the $250 to Paypal, plus whatever other Paypal fees there are. That is $212.50 not including the Paypal fee which will probably be another 2-3% so you are looking roughly at $208.25.
This is this most effective strategy that I can think of when it comes to the RMAH not including third party websites.
Edit - Forgot to mention there is a 15% fee for commodities such as gems, recipes and crafting materials. This is 15% because you can for example stack 20 of a crafting material and sell all 20 together in the RMAH.
