Check out "air video server"... I know, I know, cliche right? Its for apple devices primarily but can be hacked (I've read) to work on various free/open OS's... If you are adimantly opposed to apple stuff for whatever reason, InMethod, is open-ish and their forums are robust enough to get the idea of what they are doing in terms of transcoding and load balancing. Basically, I used it as a novice, and now I use it to check my notes while using plex running on ubuntu or as a vm on my macpro (OSX10.6 Server) which serves as my server. My job necessitates that I am on the go sometimes without warning and this is the only thing, after much trial and error, that works, every time. Specifically, their caching and transcoding workflow is well thought out because they have had to work under apples strict caching and ram using requirements specifically on the iphone 3 (when I first tried it). Looking back, how they got video (choppy and blocky as it was at times), to not drop or throw an error, all the time, and every time, is still beyond me.
No I don't work or develop for them, but I have spend a lot of time and effort to find something exactly like you are describing to no avail. I'll be checking in to see if yall find anything creative or more effictive for a more advanced setup (I'm thinking vm based on a server behind a router/switch with a static ip)... good luck.
