What brand is the card?
How much thermal paste did you use? Typically use a little bit more on a GPU than you would on a CPU.
Is the fan working correctly? You can check the RPM with MSI afterburner or GPU-Z (off techpowerup.com).
There are aftermarket fan/heatsinks, it would all depend on what card your friend has.

GPU cards do not randomly freak out... there has to be a reason.
Can I assume you are running stock clocks & volts - no OC?
If the paste is good and the HS is solidly on with no wobbles and the fans are good, it just may be on the way out.

That leaves us with fans and HS..
As Bal3Wolf suggested, the fans may be an issue. Can you check with GPU - Z and confirm temps are nominal and fan RPM is at 100%?

only thing i can think of, seeing as you have replaced the TIM etc, is that the temperature sensor is giving false readings once it heats up. what are the idle temperatures? and does the temp suddenly jump up within milli seconds? or does it gradually increase?
