Download MSI Afterburner, play some games and see how high activity it reaches. If it does not reach 99% or close, overclocking the GPU will not help. Also check that the framerate corresponds with lag.

That you hare having performance issues with Diablo 3 makes me think this is something more serious though. Your components should be overkill for maxing it out. First thing, wipe and reinstall graphics drivers. Might even have to reinstall windows, but this kind of problem obviously isn't very easy for me to figure out as it could be any number of things untill you can find out something more specific about what is causing the problems, hands on troubleshooting might be needed.

Download 3Dmark11 and post results here, the graphics and physics score might at least give some indicator on which part of your system is underperforming or wether it's something completely different.

Thats just because it downclocks when idle, probably.

Something is severely off there..
As said earlier grab something like MSI Afterburner or Sapphire TRIXX and have a quick look at what your actual clock settings are at. Those scores point towards either the card is underpowered or downlocked manually.

Screenshot wise. Just something that gives us a quick peek into your computer.
Various GPU-Z / CPU-Z / Utility screenshots or such should suffice.

memoryclock: 150 MHz
coreclock: 250 MHz

Have you tried the above and downloaded " MSI Afterburner" or " Sapphire TRIXX " and checked what the core and mem clocks are there. If they're still 250 / 150 move the slider up to the 850 (core)/ 1350 (mem) range.
