The best small mid tower in my opinion is the Corsair 300R, my case right now.
It is small enough to fit near anywhere and blend in.
Large enough to support SLI GTX680's.
Enough internal space for cable management and mounting water cooling.

Next I'd reccomend the Corsair 500R, CM 690ii, or pick your Lian Li.
I like the more minimalist style cases.

Have you taken a look at the HAF series from Coolermaster? I'm not a case expert but I can go from personal experience.

1) Plenty of room for that, the upper HDD bays are removable.

2) There's a screen for the PSU but it is a bottom mount so I don't know your thoughts on that. I'm not sure about filters for intake fans otherwise, I think there are just not so sure.

3) most of my cabling is behind the mobo, the backplate comes off with ease and there's about half an inch to a full inch for cabling back there if memory serves correctly.

4) All 200mm fan mounts support 2 smaller fans instead, does not come with any 200mms.

5) has the holes for the tubes and its sizable enough to support the H100 my friend told me when he examined it. It certainly will accept the H80 at least.

6) the removeable harddrive bays can also simply be rotated, the mounted bays are perpendicular to the GPU (you insert it facing out of the side panel)

7) my HAF 912 was under 100, the other ones aren't very expensive that I've seen.

Its not a designer case or a really special one but it gets the job done.
The other suggestions here are probably better but just thought I'd throw in my 2cents.

The HAF 912 is okay, but it doesn't quite match what I'm looking for, overall. I like it, but it's just not for me.

That said, I saw the HAF XM, and that looks like a shrunken version of the HAF X, which is a case I had been looking at before shrinking to mid tower.

I also like the look of the Corsair 300R and 500R.

Phantom 410 FTW! If i could save enough cash, I would get the Red finish with Blue lighting interior variation of this bad boy. Its a damn near perfect case for anyone IMO.

Agreed...to an extent.

@OP:
The Corsair Carbide 300R is a good, cost effective, and attactive chassis to start off as a platform. Once you've added your own intake/exhaust fans you are still generally under the pricepoint of its honest competition.

You will have to rule out the H100 as an option unless you are willing to undertake a massive (de)fabrication effort and/or one of those ridiculous exterior refits...which defeats the stated purpose of this chassis -aircooled.
The H80 will fit if you are referring to the Z77 Sabertooth, although the LGA2011 X79 Sabertooth is ruled out due to its form factor.

The optional side and top fan locations have no inherent air filters...but this is easily accomplished either with 3rd-party or DIY alternatives.

The backplate is large (too large IMO) and can actually hinder cable management -if you like me don't like the thought of drapping cables over the opening.

The top mesh insert is rather flimsy and can become distorted during fan placement -unless you are aware of this and pre-tap the fans beforehand.

I went from an old and gaudy (Read: LED festooned/stylized side window) >$200 super tower to this <$80 minimalist mid with the same internals and so far have zero complaints and absolutely no looking back.

I paid 35 bucks for that gamma! it is the same as my m59 inside. great cases for 35-45 shipped! it really is good case it puts some better known brands liek antect to shame some of there case that cost more don't even have 90 degree hard drive cages, let alone wire management.
