This was my first 'real' card and it has served me well, but it is time for an upgrade 
I bought it from another OCN member in November last year.
I overclocked it to 825 core on stock volts, I had it at 850 at 1v I think for like a week, never tried higher than that. The highest temperature I've seen on it was about 62. (Of course, as always with overclocking and temperatures, YMMV)

I don't have the box, instructions, or CD, and it come with no accessories unless you reallllllllly need a molex to 6-pin adapter I can give you one, and I guess since I'm planning on getting an AMD card I can give you an sli bridge if you need one.
