Budget ~$300
the top part is plastic wrapped in steel so the visible surface is steel with matte texture. there is a small plastic cover that slides over the USB ports but it's fairly tastefully done. quality wise, it's generally considered top of the line in the industry (tho some people have gotten lemons, but, that's to be expected with any manufacture) the chassis is steel with a 4.5mm aluminum unibody wrapped around the front/bottom/back side of the chassis. it's maybe the most classy looking case currently on the market.
Personally I use the 650D. It's a beautiful case that does everything I need and more! It was totally worth the money! I haven't used the cases that we're listed above, but I think that they are worth looking into. In the end you need to find something that you can feel an emotional attachment to. I would stay away for NZXT. The reason I would stay away from them is because they have weird fan sizes that cause a lot of turmoil. The only fans that seem to work in their cases are the ones that they manufacture. Although that has been my experience someone else could have experienced different.
Cooler master haf-x or haf-xm. my previous rig was in a haf-x and I really liked it. It fits most of your requirements as well.
